I usually document things in the build descriptions, for example:
https://gar.svn.sourceforge.net/svnroot/gar/csw/mgar/pkg/urxvt/trunk/Makefile

The patches can also be seen there, in the files/ subdirectory.  Do
you usually read build descriptions when you examine packages?
