[csw-maintainers] package request page broken

I've just tried to use the /requestPackage/ page on the opencsw.org website
and unfortunately the Send button doesn't work, so you can't submit the
form.

Also, when you click into a field, I'm assuming that it's suppose to
automatically select-all, so you don't have to manually select-all and
delete the values that are in there by default?

Not sure who would look at this.

This is happening on Chrome and Firefox on my mac. It works on Safari.
